It's a bit tough to see from the picture, but it looks like the pin is through the middle phalanx, in order to be hooked up to a "banjo split" AKA "pie tin splint" via rubber band so that traction can be applied throughout the range of motion. (The splint has a circumferential forearm base and then a hoop attached with the center of the hoop aligned with the PIP joint. On the hoop is a sliding mechanism to which the pin is attached via the rubber bands.) |

A rubber band would be hooked to the pin to give PIP distraction with a "banjo splint" so that the PIP could be passively moved thru an arc of motion. |

I have seen pins placed for traction of the PIP joint with splinting to allow motion in limited range to prevent adhesions. |
