This is what occurs when a metacarpal fracture heals with a rotational deformity. Notice the alignment and how the little finger is crossing over the ring finger and when you look at the radiograph you can see why. If you draw a line from the nail bed to the thumb you will see that the alignment is not correct as the nail bed should align with the scaphoid and not the thenar eminance.
